CAPEL DARDANUP HOME AND COMMUNITY CARE

Home & Community Care provide services to assist the aged, carers and people with a disability to stay living in their home and in their community. A range of services are available to people who live in the Capel and Dardanup shires.

If you feel you could benefit from these services, please contact us. One of our helpful friendly staff will call to your home and discuss your needs.

This service is a Home & Community Care (HACC) funded program. It is jointly funded by the Commonwealth & State Governments.

COSTS

The fee for service is based on an income assessment. If you are on a full pension, your contribution will be:

$8.00 return
$8.00 per occasion
$10.00 per hour

$8.00 for transport component and any associated cost with the outing.

TRANSPORT

Transport vehicles are in your area on a daily basis. We provide transport to doctors surgeries, hospitals, shopping, banking or to enable you to enjoy social activities. eg. at a day centre or senior citizens clubs.

Wheel chair specific transport is available, and is brokered through the Bunbury Taxi service.

DOMESTIC ASSISTANCE

Is to assist you with the heavier household jobs. eg. vacuuming, washing floors, cleaning toilets and bathrooms. The assessment process will determine if you require weekly or fortnightly assistance. Domestic assistance is generally 1 to 1 1/2 hours each occasion.

MAINTENANCE

Maintenance of the home includes; changing of light globes, gutter cleaning, fixing leaking taps and other small maintenance tasks.

GARDENING

This service combined with the maintenance service, assists people to maintain their own homes and gardens. It includes the mowing of lawns, pruning, weeding and removing rubbish to the tip.

CARER SUPPORT

This service aims to support the carer by providing an "in-house" short term substitute care, this allows the carer to pursue their own activities and appointments.

The person being cared for may also benefit from the break. Carer support can be "one-off" or on a regular basis.

SOCIAL SUPPORT

Is provided to people who are socially or geographically isolated and require individual support.
